Three Hots and a Cot
The Cot
If a character wants to sleep, they gotta have a cot. I don't care if that's a bedroll on the cave floor, a bed at an inn, or some other third bed I can't currently think of. On top of that, it's gotta be a hot cot. The best sleep is somewhere safe and warm, making that bed at the inn superior to that bedroll on the stone floor of a cave where even the air hates you.
To put it another way,
A character sleeping with a bedroll or equivalent in a safe and warm area sleeps well. They make progress on any ailments or injuries they currently suffer (or they regain XdX+X HP - whatever is a good number/range).
A character lacking either a bedroom or equivalent or a safe and warm area may sleep well or poorly. Flip a coin (or 1-3 on a d6) and have them call it. If they are right, they sleep well (see above). If they are wrong, they sleep poorly (see below).
A character lacking both components sleeps poorly. Their current ailments and/or injuries do not improve (or they regain XdX-X HP - whatever is a bad number/range). On any subsequent nights of poor sleep, their current ailments and/or injuries become worse.
Of course, other things can impact a character's sleep. A character under the curse of "run into walls while you sleep" obviously is going to have problems. Exceptions do exist - take it on a case-by-case basis.
The Hot
If a character wants nourishment, they gotta eat. Maintaining food is crucial to being alive. A well-fed character is a healthy character; a well-fed character recovers from ailments and injuries better. Since they gotta eat, they need to make sure what they are eating is safe. There's three general categories of "safe to eat":
Fresh
Food caught or harvested recently should be assumed safe to eat. This works for game and foraging, as well as markets. Fresh food can be found wherever food can be attained or harvested, but must be eaten soon for it to remain fresh.
Hot
"Hot food is healthy" - someone, at some point
Food cooked properly should be assumed to be safe to eat. This works for basically all non-poisonous foods (and can make some poisonous foods safe to eat). Hot food is most common in civilizations or in locations where the character camps for longer periods of time.
Preserved
Whether dried or salted, properly preserved food should be assumed to be safe to eat. This works for meats, vegetables, and fruits. Preserved food is better for long-distance travel and quick eating.
Other Factors
If it rained, flip a coin (or dice) and have a player call it. If they are right, the food is still safe to eat. If they are wrong, the food has gotten wet and spoils, unless the food is dealt with immediately.
If a character is carrying something cursed/magical/radioactive/funky/etc., flip a coin (or dice) and have a player call it. If they are right, food's safe; if they are wrong, the food has become contaminated and cannot be eaten safely. Maybe it comes alive or becomes sentient too, that'd be fun.
Or, to put it another way,
Safe food is properly prepared, either beforehand through preservation, by preparing it through cooking, or eating it fresh here and now. There is always a chance safe food becomes unsafe due to outside factors, such as weather or other items the character has. Maintaining the integrity of safe food is crucial for keeping it safe; being careless means going hungry or potentially food-poisoning yourself.
